By booking and attending an appointment with Stone Performance Physio, you agree to the following Terms and Conditions.

1. Appointments

  • Appointments may be booked online, by telephone, by email, or through other approved booking methods.
  • Patients are responsible for ensuring that all personal, medical and contact information provided is accurate and up to date.
  • Stone Performance Physio reserves the right to amend appointment times where necessary and will make reasonable efforts to notify patients of any changes.

2. Fees and Payment

  • Fees for assessments, treatments, rehabilitation programmes, reports and other services will be communicated at the time of booking or on request.
  • Payment is due at the time of appointment unless alternative arrangements have been agreed in advance.

Stone Performance Physio reserves the right to:

  • Refuse future bookings where fees remain outstanding
  • Withhold reports, letters or treatment summaries until payment has been received
  • Recover unpaid fees through appropriate debt recovery processes where necessary

All fees are quoted in pounds sterling (£).

3. Cancellation and Non-Attendance Policy

We understand that circumstances change and ask that patients provide as much notice as possible if they are unable to attend.

Cancellation by Patient

  • More than 24 hours' notice — No charge
  • Less than 24 hours' notice — 100% of the appointment fee will be charged

Failure to Attend (DNA)

Patients who fail to attend an appointment without prior notice will be charged 100% of the appointment fee. Repeated missed appointments may result in discharge from the service.

Cancellation by Stone Performance Physio

Where it is necessary for Stone Performance Physio to cancel an appointment, an alternative appointment will be offered as soon as reasonably practicable.

4. Assessment, Treatment and Consent

Assessment and treatment will only be provided with informed consent. Patients will be given information regarding:

  • Proposed assessment and treatment
  • Potential benefits
  • Potential risks
  • Alternative treatment options where appropriate

Patients may decline or withdraw consent at any time. Consent may be verbal, written or implied through participation in treatment.

5. Patient Responsibilities

Patients are responsible for:

  • Providing accurate and complete medical information
  • Informing the physiotherapist of any changes in health status
  • Following agreed rehabilitation programmes and exercise plans
  • Raising any concerns regarding treatment promptly

Failure to disclose relevant medical information may affect the safety and effectiveness of treatment.

6. Rehabilitation and Treatment Outcomes

Stone Performance Physio will use reasonable skill, care and professional judgement in providing physiotherapy services. However:

  • No guarantee can be made regarding treatment outcomes
  • Recovery times may vary between individuals
  • Return to sport, work or other activities cannot be guaranteed
  • Treatment success may depend upon adherence to prescribed exercises and rehabilitation plans

Patients acknowledge that physiotherapy outcomes are influenced by many factors beyond the control of the clinician.

7. Medical Emergencies

Stone Performance Physio does not provide emergency medical services. Patients experiencing severe symptoms, chest pain, sudden neurological symptoms, breathing difficulties, suspected fractures or other medical emergencies should seek immediate assistance through NHS emergency services.

8. Additional Letters, Reports and Forms

Routine clinical records form part of normal care. Additional documentation requested by patients or third parties may attract an additional fee, including:

  • Employer letters
  • School or university letters
  • Insurance reports
  • Medical reports
  • Supporting evidence letters
  • Administrative forms
  • Referral letters

Fees will be advised before work is undertaken.

9. Children and Young People

  • Children under the age of 16 must normally attend with a parent, guardian or person with parental responsibility.
  • Consent arrangements will be managed in accordance with applicable legal and professional guidance.
  • Stone Performance Physio reserves the right to require parental attendance where considered clinically appropriate.

10. Chaperones

Patients may request a chaperone for any consultation or treatment session. Where practical, reasonable efforts will be made to accommodate such requests.

11. Confidentiality and Data Protection

Stone Performance Physio is committed to maintaining patient confidentiality and complying with UK data protection legislation. Personal information will be processed in accordance with the Stone Performance Physio Privacy Policy.

Information may be shared:

  • With patient consent
  • With healthcare professionals involved in care
  • Where required by law
  • Where there is a safeguarding concern or risk of serious harm

12. Professional Registration and Insurance

Stone Performance Physio maintains appropriate:

  • Professional indemnity insurance
  • Public liability insurance
  • Professional registration
  • Continuing professional development requirements

Evidence may be provided upon reasonable request.

13. Complaints

We are committed to providing a high-quality service. If you are dissatisfied with any aspect of your care, please contact Stone Performance Physio as soon as possible so that concerns can be investigated and addressed promptly. A copy of the Complaints Procedure is available on request.

14. Limitation of Liability

Nothing within these Terms and Conditions excludes or limits liability for:

  • Death or personal injury caused by negligence
  • Fraud or fraudulent misrepresentation
  • Any liability which cannot legally be excluded

Subject to the above, Stone Performance Physio shall not be liable for indirect, consequential or unforeseeable losses arising from the provision of physiotherapy services.

15. Website Information

Information provided on the Stone Performance Physio website, social media platforms and promotional materials is intended for general information purposes only and does not constitute individual medical advice. Patients should seek professional assessment before acting upon information obtained through these sources.

16. Changes to Terms and Conditions

Stone Performance Physio reserves the right to amend these Terms and Conditions at any time. The most recent version will be available on the website and available upon request.
